We realize that no one likes to spend time filling out forms. However, there are several documents and policies that we are obliged to ask you to read and sign, including documents that allow us to collect payment directly from your insurance company, or release information to your referring physician. In addition, although we often receive documentation from your referring doctor, there is always additional medical information that our surgeons require in order to be able to make a complete and accurate evaluation of your condition, and decide on an appropriate treatment plan.

To facilitate this we provide these documents below in a downloadable form, and ask that you fill each of these out and bring them to your first appointment with us. If you are a returning patient, but it has been more than 3 years since you were at our office, we will ask you to fill them out again in case there has been any change in your information.

Again, please remember any x rays or other studies that you are asked to bring, and to have any insurance cards with you.
